The application form allows people to designate someone to submit the in-person application and pick up an absentee ballot for the voter, Czarny said.
All you have to do is print out the form, fill it out, and put the name of the designated person on the sixth line.
The Board of Elections doesn’t require any additional paperwork, such as a doctor’s note or a letter from the health department, Czarny said.
